Practicing Medical Skills in a Small Clinic
Chapter 412 - 204: The Difference Between Practice and Real Operation, A Tough Case Arrives (Part 2)
Dr. Chen wore the expression of a wronged wife.
As a doctor in the bone setting clinic, yet unable to set bones, this was his biggest frustration.
And he simply couldn’t find a solution.
"Do you practice bone setting techniques at home?"
Li Jingsheng thought teaching Dr. Chen some bone setting techniques wasn’t a bad idea. But he would need Doctor Xu’s approval for that.
Otherwise, if he taught Dr. Chen privately, it would certainly upset Doctor Xu.
"I’ve practiced! I even bought joint replicas worth over ten thousand yuan online, specifically for practicing bone setting techniques. I practice every day, you can tell by looking at my hands."
Dr. Chen spread his palms, revealing a layer of calluses, especially on his right hand.
It seemed he really practiced diligently at home.
"I don’t know if it’s my lack of talent or what? At home, I practice on the replicas quite smoothly; I can disassemble and reassemble the joints with my eyes closed. But once it’s on a patient, it just doesn’t work. Last week, you saw it too; when aligning the elbow joint of that kid, I tried twice but couldn’t get it right, and the family almost punched me."
Recalling that incident, Dr. Chen felt wronged and angry, yet helpless.
Encountering families with bad tempers, any issue during bone setting can easily result in the doctor being blamed.
"Synthetic joints only have bones, without muscles, blood vessels, ligaments, skin, periosteum, and other tissues. You can reset them smoothly because there’s no obstruction from muscles, ligaments, and other tissues. In clinical practice, it’s difficult because there are muscles, blood vessels, and other tissues, and with skin covering, you can’t see the condition of the bones. Plus, the soft tissue obstructions are significant."
Li Jingsheng enlightened him.
There’s often a big difference between practice and actual procedure.
For example, inserting an IV needle, some nurses are excellent and can do it swiftly and accurately.
Patients hardly feel much pain.
It’s like being nibbled by an ant, and the IV line is successfully established.
Some practice needle techniques on synthetic rubber blood vessels, seemingly accurately every time, but fail when it’s on a patient. The reason is the significant difference between real human tissues and synthetic ones.
"So how do I solve this problem?"
Dr. Chen asked humbly.
"There’s no other way, just watch more, learn more, research more, and practice whenever there’s an opportunity."
Li Jingsheng felt the best way to quickly improve bone setting skills was to practice on patients.
However, this faced a big challenge, as no patient wanted to be a guinea pig.
This required a mentor to stand by.
If the apprentice couldn’t succeed within two attempts, before the patient or their family got angry, the mentor would quickly take over. And say a few words to smooth things over for the apprentice.
"This joint misalignment isn’t a normal misalignment; the angle is tricky, making it much harder to reset. Certain doctors typically succeed with common patients, but yours might be too difficult. Let me try helping you reset it personally."
Saying something like this to the patient helps both the patient and their family understand.
They naturally won’t complain further.
At this point, as long as the mentor treats the patient, it’s a happy ending.
Afterwards, the apprentice needs to reflect carefully, figure out what was missed, and seriously summarize the experiences and lessons to improve next time.
Only this way can they become stronger.
Dr. Chen is quite diligent and has a correct attitude but lacks in research and understanding.
Perhaps that’s why Doctor Xu is unwilling to teach him.
"Hurry up with work! So many patients are waiting outside, what are you two dawdling for?"
Doctor Xu came in, seeing them idle made him displeased.
He scolded them sullenly.
Specifically, Doctor Xu seemed to always get upset whenever he saw Dr. Chen pestering Li Jingsheng.
Afraid Li Jingsheng would pass on the bone setting expertise to him.
"Li Li, call the number!"
Li Jingsheng urged her.
"Already called, patient number 1 is lying there, but the family is nowhere to be found."
She had already called the number.
"Then treat number 2 first!"
Li Jingsheng glanced at the patient lying on the flatbed and directly called the next one.
The doctor’s time is precious and cannot be wasted waiting for a patient.
Some families during visits don’t understand the procedure, like needing to drink more water for a clear ultrasound due to a full bladder, and then the patient empties their bladder before examination. In such cases, if an ultrasound can’t be clear, the patient would need to drink more water and come back for examination in half an hour.
The patient lying on the flatbed was an elderly person, looking quite aged.
Without the family present, the doctor wouldn’t dare treat him.
Patient number 2 was brought in by two family members.
"Oh, what happened to this leg? It’s so seriously injured!"
Li Jingsheng noticed the patient’s right leg, from thigh to ankle, was severely bruised and swollen multiple times.
The thigh was almost as thick as the waist.
The injury looked truly frightening.
"Don’t even ask, got accidentally crushed while unloading goods on the leg. Luckily, dodged quickly, only hitting the right leg. If it had hit the body, he might not be here."
The family was a man and woman.
The woman was over forty, with large feet and hands, a broad back, and a thick waist.
Clearly a woman who performed heavy manual labor often.
